A One-Page Profile is a best practice tool first developed by Helen Sanderson Associates in the UK and is now in use internationally.

One-Page Profiles are the foundation of personalisation and can lead to positive change for people, whatever their age or circumstances. They provide us with an at-a-glance way of knowing what really matters to people, which can be taken with them as they move through services and come into contact with people.

For older adults in your care, it provides a record of how they'd like people to help them, including if and when they need care and support. And is a proven tool for operationalising personalisation consistently and measurably.

Audience

The course is for anyone working in long-term aged care, specifically working in dementia care, who want to improve intake, personalise care day-to-day, and improve all areas of supportive care. This includes all staff and caregivers, such as activity professionals, personal support workers, behavioral support, and volunteers.


When

 The workshop is three hours in total, split into two 90-minute sessions, which take place two weeks apart. There will be a maximum of 10-12 participants per workshop.

During this session, you will learn how to do each of the three components of a One-Page Profile well.

Between the two sessions

You will be asked to gather information in preparation for completing a One-Page Profile.


Session two of two

During this session, you will use the information you gathered ahead of the session to:

  • Complete a One-Page Profile
  • Give and Receive Feedback on improving the profiles
  • Discuss what we foresee putting into practice based on each profile

Outcomes

Following this masterclass, you will be able to implement One-Page Profile best practices into your care provision. This means people in your care will be exercising more voice, choice, and control in their care provision. This will lay the foundations for better care planning, narrative care, and improve relationships with family, volunteers, and the wider community.

Instructor

Carolyne Taplin

Carolyne Taplin is the course instructor for Lifelong Inspiration One-Page Profiles Masterclasses in Australia and New Zealand.

For the last ten years, Carolyne was a trainer in person-centred practices for Dementia Australia and the Eden Alternative.

In a long and varied career beginning in New Zealand Carolyne worked for CCS Disability advocacy and support and then taught ESOL and Literacy in universities, polytechnics, and local halls.  She has delivered workplace training in wineries, plastic factories, auction houses, dairy processing plants to roading gangs, machinery operators, security guards, and carpentry students.

For a period Carolyne had her own business working with people who live with cognitive changes, such as intellectual disabilities and dementia, supporting social interaction and well-being.

In 2013 a lifelong interest in other cultures and languages led to a move to Australia where she had a short stint writing training material for a Registered Training organisation before realising she really missed students.  Since 2013 and following her mother's diagnosis of Alzheimer’s Disease and Vascular Dementia Carolyne,  gained qualifications in the Montessori Technique as part of a cognitive rehabilitative Therapy model she then delivered education and training for Dementia Australia and the Eden Alternative.

Her work has focused primarily on Dementia Education and Culture Change for health professionals and families, promoting care models over “a cure.” 

Through her work with One-Page Profiles she believes that we can change the way we connect, relate, and celebrate who we are and who we all becoming.

When she is not teaching Carolyne can be found wandering the coast and bush in Southland, New Zealand, taking photographs or jamming on ukulele and African drums with her children and grandchildren.
